2011-2012 CONCERT BAND REGISTRATION FORM
This form is only for High School students who are in one of the band
classes, concert band, symphonic band, jazz band or percussion class. Click
the icon above to view and print this form. Place completed forms in the
Band Booster box.
MEDICAL & EMERGENCY CONTACT FORM
This form is needed only for students not in Marching Band and is used
for Spring band trips. This
provides the necessary emergency contact and medical information. Click
the icon above to view and print this form.
ONLINE PAYMENTS
Parents can pay student assessments with a credit card or PayPal. Click

PAYMENT ENVELOPES
When turning in assessment payments or other funds, please use the booster payment envelopes (see sample below) that are available on the booster
box in the band room.
