MARCHING BAND Now Available

New for 2011: The forms can now be filled out on your computer! Left-click the link above to open the document or right-click and select "Save Target/Link as" to download to your computer.  After downloading, just open the file and start typing. Enter your information, then print and sign (you can also write in any info). Hint: use the Tab key to move to the next input field.  Only submit the forms pages, keep the letter and Assessment Policy pages.

2011-2012 CONCERT BAND REGISTRATION FORM

This form is only for High School students who are in one of the band classes, concert band, symphonic band, jazz band or percussion class.  Click the icon above to view and print this form.  Place completed forms in the Band Booster box.

MEDICAL & EMERGENCY CONTACT FORM

This form is needed only for students not in Marching Band and is used for Spring band trips.  This provides the necessary emergency contact and medical information.  Click the icon above to view and print this form.

ONLINE PAYMENTS

Parents can pay student assessments with a credit card or PayPal.  Click here for more information.

PAYMENT ENVELOPES

When turning in assessment payments or other funds, please use the booster payment envelopes (see sample below) that are available on the booster box in the band room.
